In the  'Comments' section of the form below, please enter the following information:

Show; role(s) you are playing, and the following:

WOMEN: please list your sizes for     dress,    blouse,     skirt/slacks,     shoes.
MEN: please list your sizes for   shirt (neck & sleeve, not just 'XL'),   pants (waist and inseam),   suit,   shoes.

All cast members, please list any costume items that you already own as well as any physical characteristics, tattoos, etc. that may affect the costumes you are assigned
